In submitting an application to FHL, researchers, course faculty, and TAs agree to follow all research procedures as detailed in the menu items to the right.  The text below is part of the application.


FHL RESEARCHER AGREEMENT


Collecting & Animal Care

I will follow FHL policies for collecting and transporting organisms, and will report my collections online. If I am a course instructor, I will ensure that all course collections are reported.

I will follow UW Animal Care Procedures for working with vertebrates or cephalopods, including the submission of required protocols that may take more than a month for approval.

Lab & Chemical Safety

I will follow all UW Environmental Health & Safety (EH&S) requirements for research safety (see Lab Procedures), and will ensure that any students or assistants working with me are following these requirements, which include being appropriately equipped and trained for the chemicals and procedures being used.

I understand that course students are not allowed to pick up or return chemicals from/to the Stockroom. If I am a course instructor, I or a co-instructor will check out chemicals from the stockroom and deliver them back to their places after use.

I will keep all chemicals in containers that are properly labeled per UW EH&S requirements.

I will have on hand Standard Operating Procedures (SOPs) and a Safety Data Sheet (SDS) for each chemical in my research space.

If I bring, send, or order chemicals to FHL, I will report them online when they arrive. Upon my departure, I will take/ ship them home, or will hand them to the FHL Stockroom Manager together with Standard Operating Procedures (SOPs) and a Safety Data Sheet (SDS) for each one.

I will make every effort to minimize the generation of hazardous wastes, and will follow the required Hazardous Waste Procedures for any I do generate.

Upon Departure

I will return equipment, materials, supplies, and live organisms to where they came from, leaving NOTHING behind in/on FHL research spaces I used, including: refrigerators/freezers, sea tables/tanks, counter tops, cabinets, shelves, and outdoor areas.

I will not leave chemicals in a research space or with another researcher at FHL.

If I am a course instructor, I will coordinate the cleaning of the lab building and confirm that all processes noted above are completed before the students depart.

I will fill out and sign a Lab Check Out form for the lab spaces I (or my course) used, and deliver the form to the FHL Front Office.

Consequences of Violations

I understand that if additional labor is needed to clear out a lab space, or to manage chemicals that were not properly labeled, documented, or disposed of, I will be charged at a rate of $50+tax/hour.

I understand that the University of Washington reserves the right to restrict access to FHL facilities for anyone who has been found to have violated this agreement.
